I have exactly the same issue with my 2000 Accord V6 — and it seems only to occur on a hot weather day; intermittently.
I’m going to try the vacuum-less fuel injector cleaner trick.

@lothian did you ever find the solution? Would love to hear from others who solved this frustratingly difficult troubleshooting issue – especially since it only happens once in a while; the rest of the time she runs perfectly!
