If it was mine, I would proceed with cleaning the MAF sensor, and checking for vacuum leaks, which is what you seem to be leaning towards next. Try wiggling the intake tube and see if you notice a performance difference, and watch Eric’s vid on looking for vacuum leaks.

January 11, 2012 at 11:00 am #445447A vacuum leak may still exist with out setting a code. The long term fuel trim would probably be over 20% before setting a code. I have been trying to find one and will eventually give in and see if I can find some one who has a smoke machine.
January 12, 2012 at 11:00 am #445448Does the truck idle high or low and does the temperature affect the idle issue such as high idle till the engine warms up?
Does this issue stay when you move from Park to Neutral? And is the idle ‘loopy’ or just 1 consistent speed?
What brand of plugs did you use?
You also may want to consider a cylinder balance test to see if you can isolate the cylinder if its a misfire issue. Here is Eric’s video showing how to manually do this test without a scan tool.
