2004 pacifica idle issue

I recently changed the spark lugs in my 2004 pacifica and while I had the throttle body off cleaned the butterfly. When I restarted it would not idle and determined that the idle air control valve was bad and replaced it. My issue now is that it starts up and idles fine when cold but after it starts to warm up and drive to the store when I come out I have to give it some gas before it will start and idle. It will sometimes also die when letting off the gas, like when coming up to a stop sign, but does not do it all the time. Any suggestions
