My moms 04 civic VP was making a weird noise Sunday. After further inspection, I noticed a small hole in the lower tb cover and a piece of the tb popping out while the engine was running. The belt was replaced with a oem belt, water pump about 25k prior.

I picked up a new tb, lower cover and a tensioner spring.
I started tearing everything apart and when I got the cover off, this is what I found.

The water pump was tight, along with the tensioner. The spring seemed decent, but replaced it anyway. The bolt in the tensioner wasn’t very tight when I took it off. Could that have caused this?

Yes, from the design of that motor a overly loose belt might walk froward enough to get torn up on the crankshaft timing notches.

That woulda came in handy last night. I’m not to familiar with the auto tensioners. But I took the bolt out, replaced the spring and reinstalled the tensioner the same way it was before. I hope that’s the correct procedure.
