2003 Dodge Grand caravan shift hard

My caravan does this alot at low speed like let of the gas it will shift hard or give it gas and shift hard all under 25 miles hr. Any ideal on where I can look to find the problem.

check your fluid level and condition.

Also check all your transmission and engine mounts

+1 on the above posts but know that you may have a transmission problem in the end. If you have a check engine light you might also want to pull that code and diagnose it to see if it has any connection to the transmissions shifting.
